This Notice describes how medical information about you may be used and disclosed and how you can access this information. Please review it carefully.

The Health Insurance Portability and Accountability Act of 1996 (HIPAA) is a federal law that requires all medical records and other individually identifiable health information used or disclosed by this practice—whether electronically, on paper, or orally—to be kept confidential.

HIPAA gives you important rights to understand and control how your health information is used. It also provides penalties for covered entities that misuse protected health information. In compliance with HIPAA, this Notice explains how we maintain the privacy of your health information and how it may be used or disclosed. 

Uses and Disclosures of Protected Health Information
We may use and disclose your protected health information for the following purposes:

- Treatment
Treatment includes providing, coordinating, or managing health care and related services by one or more health care providers.
Example: Providing a consultation or evaluation.

Payment includes activities related to obtaining reimbursement for services, confirming coverage, billing, collection activities, and utilization review. 
Example: Submitting a bill to your insurance company.

Health Care Operations
Health care operations include the business aspects of running the practice, such as quality assessment and improvement activities, auditing functions, cost-management analysis, and customer service. 
Example: Internal quality review. 

We may also create and distribute de-identified health information by removing information that could identify you. 

We may contact you to provide appointment reminders or information about treatment alternatives or other health-related benefits and services that may be of interest to you. 

Any other uses and disclosures will be made only with your written authorization. You may revoke such authorization in writing, and we will honor that request except where action has already been taken in reliance on the authorization. 

Your Rights Regarding Your Health Information
You have the following rights with respect to your protected health information. These rights may be exercised by submitting a written request to the Privacy Officer: 

- The right to request restrictions on certain uses and disclosures of your health information, including disclosures to family members, close personal friends, or others you identify. We are not required to agree to all requested restrictions, but if we do agree, we will comply unless you revoke the restriction in writing. 

- The right to request confidential communications by alternative means or at alternative locations.

- The right to inspect and obtain a copy of your protected health information.

- The right to request amendments to your protected health information.

- The right to receive an accounting of disclosures of your protected health information. 

- The right to obtain a paper copy of this Notice upon request. 

Our Responsibilities 
We are required by law to: 

- Maintain the privacy of your protected health information.

- Provide you with notice of our legal duties and privacy practices regarding your protected health information. 

- Abide by the terms of the Notice of Privacy Practices currently in effect.

If you believe your privacy rights have been violated, you may file a written complaint with this office or with the U.S. Department of Health and Human Services, Office of Civil Rights. 

We will not retaliate against you for filing a complaint.
